Case managers here carry small caseloads on purpose. When you call, a person who knows you answers. That is the whole model, and it works.

What to expect

    You are matched with a case manager. They learn your story once, and you never have to retell it from the beginning.

    Housing applications, benefits paperwork, rides to appointments, medication routines. The unglamorous work that keeps life steady.

  3. Visits that come to you

    Your case manager meets you at home, at the library, or wherever works. Support happens in your life, not just in an office.

  4. A long-term relationship

    This is not a 30-day program. Case management continues as long as it helps, and quiets down as your own footing gets stronger.

How to start

Call (757) 555-0126 and ask for community support intake. If you are being discharged from a hospital, ask your discharge planner to send a referral. We schedule the first visit before you leave.
